Accountable Care Case Studies

Physician groups and health care systems across the nation have successfully managed chronic diseases, developed effective care transition and care coordination processes, and implemented many patient-centered programs that work to improve quality and contain costs. Rather than reinvent the wheel, CAPP believes that as the nation moves towards delivery system reform based on outcomes, much can be learned from work already done to date by these organized delivery systems and physician groups. Through its Accountable Care Public Education campaign, CAPP is collecting and sharing case studies from health systems across the country.
